Once you fill up this form and submit it, you would get a call from our customer service team to confirm the address and the tests that need to be done. They would explain the preparation for the test and also inform you the time when the phlebo would be arriving at your place. The sample collection would be free of cost if the amount of the tests is more than Rs. 500 and your place is within 5 kms from our laboratory. For tests which cost less than Rs. 500 or if your place is more than 5 kms from our centre, the charges would be Rs. 50 for sample collection and Rs. 10 per km as travelling charges.
